Evidence also indicates that police may feel less confident, after training, in administering naloxone than other groups 45. They often tell me they’ve adapted to the risks of fentanyl by smoking rather than injecting the drug, which many addiction experts believe is safer (though still incredibly dangerous). People try to never use fentanyl alone and often carry naloxone or Narcan to reverse overdoses. Many people use test strips to identify unwanted contaminants in their drugs and use smaller fentanyl doses. In most of the the U.S., gangs are selling complicated “cocktails” of street drugs.
TRENDS IN MORTALITY DUE TO DRUG POISONING AND ALCOHOL

High and Rising Mortality Rates Among Working-Age Adults.

Results of Study on U.S. Life Expectancy
Among females, the average annual number of deaths from excessive alcohol use increased by 15,136 (34.7%), from 43,565 during 2016–2017, to 58,701 during 2020–2021. Age-standardized alcohol-attributable death rates among females increased from 22.7 per 100,000 population during 2016–2017 to 23.6 during 2018–2019, and to 29.4 during 2020–2021. Death rates among females were highest from heart disease and stroke during each period. Among both males and females, alcohol-attributable death rates increased for most cause of death categories. The average number of sex-specific alcohol-attributable deaths increased among all age groups from 2016–2017 to 2020–2021(Figure). The average annual number of deaths from excessive alcohol use among males increased by 25,244 (26.8%), from 94,362 deaths during 2016–2017 to 119,606 during 2020–2021 (Table 2).
When follow-up was prospectively scheduled, the interval ranged from two to 12 months, with a three-month interval utilized most frequently 27,28,29,30,31,32 The median proportion of participants with follow-up data was 10.5% among 18 studies that used passive follow-up but 74.5% in 12 studies with prospectively scheduled follow-up. Four studies, by design, included only participants with follow-up data, and follow-up was not described in the seven police studies. On the positive side, 53% say drinking alcohol increases their enjoyment of food and meals either a lot or a little. A majority of drinkers (57%) think their own alcohol use does not increase their risk of serious physical health problems. Still, 43% say their drinking does increase their risk of health problems either a lot or a little. These trends do not significantly reflect changes in violent crime or gun violence.
The White House has put forth model naloxone access laws for states to consider 73 and has met with naloxone manufacturers to urge them to increase naloxone access and affordability for at-risk communities and organizations serving high-risk individuals 74. The Substance Abuse and Mental Health Administration funds states, tribes, and communities to purchase naloxone and develop plans to saturate their communities with naloxone. We included only studies that provided descriptions of the study population, the intervention(s), and our primary outcome measures. We excluded studies of interventions based in healthcare settings such as the emergency department and pharmacies, solely qualitative studies, reviews, case studies/series, dissertations, letters, and editorials. A national survey reporting overdose outcomes from OEND programs across the United States 13 was excluded because some of the data duplicate those reported by some studies in this review.
